A cover-up is a new design built over an existing tattoo so the old one stops reading. It is a design problem more than a tattooing one: the new piece has to place its darkest areas exactly where the old ink is heaviest.
Not always, and it is better to hear that early. Size, age, and how dark and saturated the existing ink is all limit what is possible — a heavy solid-black piece restricts the options far more than a faded fine-line one.
